"The right posterior hepatic notch sign is a sharp indentation of the normally smooth posterior right hepatic lobe margin. It is reported to be highly specific for advanced cirrhosis and can be associated with an expanded gall bladder fossa. Caudate lobe hypertrophy and lateral left segment together with atrophy of the right posterior hepatic lobe are thought to generate this appearance 1."
